Health Center: New Patient Procedures
Call the Health Center or walk in during our normal hours of operation. An intern is always available and will be assigned to your care under the supervision of one of our licensed Faculty Doctors. If you have been referred to the clinic, you may ask for the intern of your choice, who will then contact you to set up a convenient time for your first visit.
Your intern will take an in-depth patient history and perform a complete chiropractic physical examination. You and your intern will then discuss your exam results and develop a program of care individualized to your needs.
Your care is monitored and reviewed by faculty doctors, and licensed D.C.s are always available for consultation, if necessary.
Getting Started...
On your first visit, you will be requested to fill out a form that includes your name, address, biographical data, as well as your billing information. Please bring a picture ID, social security number, and be prepared to make payment for all services at the time of visit. You will also be given a copy of the fee schedule that you sign and agree to, as well as our privacy notice. Paperwork takes approximately 30 minutes to complete.

When finished click on the “submit” button at the bottom of the page. (Depending on the choices made when completing our privacy documents, the last two pages may or may not be active. These only need to be completed if they are active. If you have any questions we will certainly be sure to answer all when you arrive for your first visit). Fees
Physical examinations $65, office visits $25. A full fee schedule will be given to patients as part of their new patient paperwork. We are committed to offering affordable services. Many insurance companies reimburse for chiropractic care. We also accept third-party pay, worker's compensation and personal injury claims.
